Our forms are in Adobe Acrobat pdf format. If you have a version of Acrobat Reader that is older than Version 5.0 then please visit the Adobe site here and download the latest Free Reader.
NOTE: The fillable forms will not allow you to save the form with your data in it, however you can save the form and print it and complete it by hand or you can print the completed form and save a printed version.
Also note that Acrobat uses popup windows to display forms. If you are experiencing difficulty with the forms and have popup blocking enabled, try disabling your popup blocker.
Reader 7 and above:
1. If you attempt to open a form and Acrobat opens, but then displays a blank:
Open Reader and go to Edit/Preferences/Internet. If the "allow speculative downloading in the background" box is checked, un-check it. Try to open the form.
Reader 5.0 and above
Open Reader and go to Edit/Preferences/Options. If the "display pdf in browser" box is checked, un-check it. Un-check any other boxes that are checked in the Web Browser Options portion of the dialog box. Try to open the form.
2. If you attempt to open a form and Acrobat does not open, make sure you have your popup blocking disabled. If it is disabled and Acrobat still does not open, please visit the Adobe site and download the latest version of the free reader at the link above. The latest versions will often fix glitches and are also equipped with better security features.
Try to right click on the form link and choose "save as". This will allow you to save the form to your own pc and then open it. This can sometimes solve problems related to connection speed, timeouts, and the like.
