Subject: Physical therapists and physical therapists
assistants
Comment: Applicants shall have graduated from an
accredited physical therapy or physical therapy assistant education program and shall have
passed the appropriate national licensing examination. Applicants shall complete a
Jurisprudence Questionnaire addressing the Arizona Physical Therapy Practice Act.
Applicants previously licensed in other jurisdictions are required to arrange for
verification of such to be submitted to the Arizona Board by the jurisdiction(s) on the
applicants behalf. Foreign educated applicants are subject to additional
requirements including a credentials evaluation, passing scores on English proficiency
examinations, verifications, of approval to reside and seek employment in the United
States. The licensure period is 2 years (September 1 of even-numbered years through August
31 of the next even-numbered year).
