Subject: Drug wholesaler

Comment: Application for Wholesaler’s Drug Permit. Required for both prescription and non-prescription medications.

Subject: Drug manufacturer

Comment: Application for Manufacturer’s Drug Permit.

Subject: Over the counter drug outlets, non-prescription medicine, vending, machines, retail, wholesale, manufacturer (requires approval by the board at a scheduled meeting)

Comment: Application for permit to sell over the counter items, i.e. laxatives, cough and cold items, etc. Two categories of permits available based on number of different medicine items stocked.

Subject: Compressed medical gas distributor, supplier

Comment: Application for registration in either category.
