Subject: Practice as an Optometrist

Comment: Applicants shall have graduated from an accredited optometry school and passed a relevant National Board of Examiners in Optometry (NBEO) exam at time of original licensure. All applicants are required to take and pass a jurisprudence exam addressing the Arizona Optometric Practice Act. Applicants graduating from an accredited optometry school within the last five years may submit a Regular application. Those currently practicing at least four out of the last five years may submit an Endorsement application. Applicants must be fingerprinted and possess good moral character.

Subject: Dispense replacement-soft contact lens from out-of-state domicile

Comment: Registration of nonresident replacement contact lens dispensers. Registrants must complete an application and identify a qualified person able to dispense contact lenses to consumers holding a valid prescription.
