Comment: Permit required of all swine feeding operations utilizing garbage with meat scraps as a feed; $5.

Subject: Handlers of dead or deceased livestock or poultry or their products direct or indirect, includes but not limited to: brokers, jobbers, pet food manufacturers and poultry processing.

Comment: License required to engage in utilization of these products.

Comment: License required for anyone who operates a fish farm, transports live aquatic animals, processes Aquaculture products or operates a fee fishing facility; $100. two year license available.

Subject: Restricted use pesticides; use or application of and/or the supervision in the use or application of pesticides

Comment: Application for annual certification credential (qualification for commercial applicators by CORE examination in specific areas of expertise). Six hours continuing education credit or retest for annual renewal. Private applications by examination - three hours continuing education credit for annual renewal; $50. Expires 12/31. Two year license available.

Subject: Sales of pesticides

Comment: Seller’s permit application form; $100. Expires 12/31. Two year license available.

Subject: Users of pesticides

Comment: Grower’s permit application form; $20. Expires 12/31. Two year license available.

Subject: Custom applicators of pesticides

Comment: Anyone who makes an application of pesticides for hire or by aircraft (also requires Commercial Applicator Certification). Application for Custom Applicator License - Copy of FAA Certification for aerial applicators - proof of financial responsibility, in the form of a certificate of insurance, written Custom Applicator CORE examination plus ground or air permit; $100. Expires 12/31. Two year license available.

Subject: Application equipment

Comment: Any aircraft or piece of ground equipment used in the custom application of pesticides. Application for equipment license tag(s); $25. Expires 12/31. Two year license available.

Subject: Agricultural aircraft pilot

Comment: Application for Agricultural Aircraft Pilot’s License - valid FAA Commercial Pilot’s Certificate Number: Written examination; $50. Expires 12/31. Two year license available.

Subject: Agricultural pest control advisor

Comment: Anyone who offers recommendations concerning the control of any specific agricultural pest condition. Application for Agricultural Pest Control Advisor - one year Ag-related experience written examinations on laws and rules and any specific area of expertise - six hours continuing education credit for annual renewal; $50. Expires 12/31. Two year license available.

Subject: Distribution of pesticides

Comment: Application for registration of pesticides; $100 per product paid annually. All companies who distribute pesticides in Arizona and whose name appears on the label of the pesticide must annually register the product with the Department of Agriculture. Expires 12/31.Two year license available.

Subject: Distribution of commercial fertilizer

Comment: Applications for commercial fertilizer license; $125 annual license fee. Application for specialty fertilizer registration; $50 per brand and grade of specialty fertilizer paid annually; $0.20/ton/quarter tonnage fee with a $2 minimum. Expires 6/30. All persons who manufacture or distribute a fertilizer in Arizona must annually secure a license and/or register specialty fertilizers with the Department of Agriculture.

Subject: Distribution of commercial feeds

Comment: Application for commercial feed license; $10 annual license fee; $0.25/ton/quarter tonnage fee with a $2 minimum. Expires 6/30. All persons who manufacture or distribute feed in Arizona must annually secure a license with the Department of Agriculture.

Subject: Sale of seed

Comment: Application for Seed Dealer’s License; $25 annually. Expires 6/30. All persons selling or distributing seed in Arizona must annually secure a license with the Department of Agriculture.

Subject: Labeling seed for the purpose of selling or to transport for sales into or within Arizona

Comment: Application for a Seed Labeler’s License; $40 annually. Expires 6/30. All persons whose name appears on the label of seed sold or distributed in Arizona must annually secure a license with the Department of Agriculture.

Subject: Removal and transportation of protected Native plants/wood

Comment: Anyone who takes, transports, digs up, or destroys any protected native plant/wood/fiber. Application for removal of native plants/wood; $5 permit fee; $6 Saguaro tag, $4 other plant tag, $0.50 minor plant tag. Application for annual salvage assessed permit; $25 permit fee; $4 tree tag. Application for an annual harvest restricted permit; $25 permit fee; $4 cord of wood, $4 ton/month following harvest.

Subject: Dairy farms and facilities

Comment: Permit required to produce raw milk for pasteurization. Annual; expires 12/31.

Subject: Cheese and ice cream plants

Comment: Application for license/renewal for manufacturing milk processing plant is required. Annual; expires 12/31.

Subject: Wholesale distributors and producer distributors, fluid milk, manufacturer of trade products, and milk sampler

Comment: Application for license/renewal is required. Annual; expires 12/31.

Subject: Shell eggs and egg products

Comment: Any person or firm that contracts or obtains possession of any eggs or egg products for purpose of candling, grading, or wholesaling to retailers or food service firms is subject to the license and fee. Retailers are exempt from licensing unless they do not buy the eggs or egg products from a licensed dealer; $25. Annual; expires 2/28.

Subject: Equine Owners

Comment: Individuals who own a horse may obtain an ownership card, good for life of horse. Application, $10.

Subject: Slaughter of livestock (cattle, sheep, goats, etc.) or poultry annual

Comment: License and inspection program required for meat and poultry products for human consumption; fees vary according to volume; $5 - $80 expires 12/31.

Subject: Processors of meat and poultry

Comment: License and inspection program required for meat and poultry products for human consumption sold at wholesale; $10. Annual; expires 12/31.

Subject: Use of livestock brand

Comment: An Arizona recorded brand is required for all range livestock; $15 - $75.

Subject: Self Inspection booklets

Comment: Allows movement of livestock in state. Dairy & Feedlot are $0.20/head plus $1.00/head Beef Council fee. 7C's are $1.00 Beef Council fee if change of ownership. (Change in ownership normally requires Animal Health Officer involvement.)

Subject: Interstate Shipment of Native Plants

Comment: If protected plants are being shipped out of state, they shall also bear a certificate of inspection by the department; $5 inspection fee

Subject: Native Plant Green Seal Movement Permits

Comment: Anyone importing native plants into Arizona must have permits certificates from Arizona if their state does not have a comparable program. $5 permit application fee, 10 cents/seal.

Subject: Agricultural Laboratory Certification

Comment: Certification is required of private laboratories providing guaranteed analysis for aflatoxin in animal feeds and whole seeds. Other certifications for lab services are available. Initial certification fee is $200 and valid for one calendar year. Recertification fee is $100 and valid for one calendar year.
